Tax deduction for medical purposes isn't a loophole, it's a deduction.

Whatever label we use, they are both allowances codified in the law. We're talking about when one should take an allowance to which one is entitled by law and when one should nobly or ethically decline it. I'm having trouble finding a rational bright line there.
