I think the Vietnam's government had reason to exaggerate the total. Even if they are not outright lying they would be likely (esp. in the years soon after the war) to look at the data in ways that makes the civilian deaths in the north seem larger, and the possibility of an outright lie is not insignificant.

Most of the people who fought, died, and killed in the war where not Americans, so a lot of the killing was done by non-Americans. Overall it seems very unlikely that we killed millions, or course hundreds of thousands is still a lot of death, and your probably not talking about the lower end of the "hundreds of thousands" range.

Another thing to remember is the United States didn't start the war, although I suppose it would have been over sooner (the South would have been defeated sooner) if we had not gotten involved, so there might have been less death.
