Apparently, 'participating in direct or indirect combat operations' is generally sufficient.

Also: in all wars, throughout time... the more unpopular the war with the troops, the more liberally medals are given out (as a way of promoting morale).

The Caesars did it, Napoleon did it, Hitler, Stalin, etc., etc., and we did it in Vietnam too.

But there is no disputing the circumstances of Bob Dole's first Purple Heart... he wrote in his autobiography that a grenade he threw landed a little short, and he got a 'scratch... the likes of which were generally treated with mercurichrome....'
