I've looked at that film several times and notice that Kerry began to withdraw the notes with his back to Lehrer and the audience and had them on the podium before turning so Lehrer could see his hands.

That smacks of detailed rehearsals of the move. He would expect to be filmed from the front. I doubt he ever expected a camera behind him to be filming the move. I doubt he even knew the camera behind him was there.
