russett,

<<Many of these studies do not correct for the fact that the people they are studying have a long history with older
NSAIDS, so are in a frail state to begin with.>>

This should NOT confound the Clevland studies IF the case/controls were set up properly. So I think this point is nonsense. Both groups should have had an equal number of frail people.

<<If you want fairness, you will want to cite a study that follows patients who have only taken Celebrex for their arthritis symptoms, against patients that have only taken the old NSAIDS.>>

When some of these studies first started to emerge, the feedback from the Mercks and Pharmacia's was that the additional heart attacks were likely due to the protective effect of the older NSAIDS they were being measured against -- NOT that their Cox-2s were causing heart attacks.

I submit the best study would be an equally matched group of those who take Celebrex and those who never take any NSAIDS (or very infrequently). Then you have a baseline that measures what you really want to know -- how many heart attacks does Celebrex cause, if any AGAINST those who do not take them.

Vioxx. Celebrex and Bextra are all Cox 2s and I am willing to bet that at some point in the future they are going to be found to be a problem.
