I've often wondered whether some of the differences in health care treatments and outcomes by class and race are simply a factor of some patients being more comfortable advocating for themselves and others simply being passive "doctor knows best" consumers.

I've generally taken an active role in my own healthcare (i.e., mostly have had my doctors well trained to do what I want <g>) but a few years ago I got to see the other side of the argument. I have an academic-type dentist and he asked me some technical mumbo-jumbo question about what type procedure he should do on a difficult tooth - I of course looked blank and responded with a "use your best judgment" spiel. <g>
