The commentator missed the most likely possibility, imo: Seymour Hersh was being had. Again.

We all know how gullible he is, how many stories he's written up that turned out not to be on the level. So who better to give an unsubstantiated atrocity story to, if you are an anti-war activist, say? Isn't that just the sort of story Hersh would like to report? All you have to do is sound plausible when you tell him, no, no, you can't give him any hard evidence, it's far too risky. He'll run with it.

Heck, he's probably spreading it around meetings because his editors all told him no way were they going to print it without some decent corroboration.
