It's easy to make a copy and paste error.

Most people perform the copy function by using their little finger on their left hand to press the cntl-key. That is ususally the weakest finger. If they then hunt for the 'C' and roll their left index finger toward it, the little finger often rises up and only the 'C' is seen by the computer.

This would leave whatever was already in the clipboard to be pasted. The Paste operation is self correcting, in that if it doesn't appear to work people will repeat it since they are looking at the screen and expecting it to change.
