A couple of points:

The MDs could have been lying in order to head off comments about AIDS. Arafat was a voracious and aggressive homosexual at a time when the likelihood of infection was high. Plus, all his travels may have led to sex with any number of people including, presumably, some sub-Saharan Africans whose rate of infection is known to be high.

Second, there is a high political interest in keeping the lid on Arafat's homosexuality, a serious Islamic sin. If Sharia were to be applied to Arafat, he could have been lawfully executed.

Too bad he wasn't killed in a martyrdom operation so he could have had access to the 72 dark eyed virgins. It would have been pure torture for him, especially since the writings indicate that martyrs are to remain eternally priapic. He would have presumably been more attracted to the young boys who assist the virgins that are also part of the shaheed deal.
