In an ethics culture governed by appearances rather than substance, no one is ever held clearly accountable for anything.

I always thought that the appearance standard worked well for accountability as regards actual violations. If there's an actual violation but they can't meet an evidentiary standard, they can always get you for appearance. The appearance standard is not always enforced but it aids in enforcing the more serious stuff.
