Well, OK. So the insurance companies avoid paying for some drugs for pain. With the pain no longer made tolerable by medicines, patients go to their doctors to find, and treat, the causes of pain. Then -- diagnosis, surgery, more expensive targeted medicines. At the end of the day, do insurers come out ahead?

Partly being argumentative, Peter; heuristically so, I hope. Personally, I have never understood the insurance industry's reluctance to pay for the cheapest treatment modality.
