Solon, that NEJM article hits on something that so many overlook, that being the overhead created by the myriad insurance plans and the paperwork they generate and the mind-boggling overhead this situation creates, eating up health care dollars that could be better spent elsewhere in the system.

A trauma center located in a resort area comes to mind. People from all over the world come to the resort. If a tourist becomes sick or gets injured and requires medical care at the trauma center, all the center needs to do is to bill the tourist's homeland government for the full freight, and the funds are forthcoming. No questions. No endless rounds of back and forth paperwork and phone calls. No food fights. - Holly
