Programs like Medicaid are fairly easy to means test because there aren't a lot of applicants and virtually all the applicants qualify or they wouldn't bother applying. OTOH, there are tons of Medicare patients most of whom would expect at least a partial payment from Medicare. You could save money by making partial payments to the more affluent but there would be a lot of overhead in it. And the burden of assembling the financial records to apply would fall to old people who often have trouble managing such tasks due to failing capabilities.

I think it's worth analyzing and costing out but I don't know how promising this is.

There's also the problem that, if Medicare payments to better-off seniors are reduced, the seniors won't end up paying the difference. Their health insurance will, which still burdens the health care system, just another part of it. What goes around, comes around.
