Permanent bans simply mean (usually) that the banee has been so nasty or disruptive that the person banning them doesn't want...

This assumes the person doing the banning is faultless. There are many threads on SI where the banning is arbitrary, or done with vindictiveness. That's another reason all bans should expire after a period of time. A life sentence for a drug dealer is a fairer sentence than a lifetime ban because of vindictiveness.
