There are a few glitches which happen due to users doing the wrong thing. As with anything, if not done right, it won't work.

I've found that having an overloaded computer or data connection causes problems, meaning lots of simultaneous processing can mess it up. If people don't have echo cancellation microphones or otherwise get feedback loops, it messes it up [like a normal telephone on speakerphone is horrible and echoes].

A normal telephone setup, namely microphone and ear-piece is the way to use Skype to get really high quality voice. A fast internet connection helps too. Then, it's great.

I use a good quality head-set, with ear-phones over both ears and a boom microphone, then it's hands-free, comfortable, with perfect sound and zero cost for Skype to Skype users.

I haven't used the 2c a minute service to PSTN phones, but I gather the quality isn't as good because of the PSTN system.
