Likely the 'income effect'. Lower income brackets have poorer health care. (Sounds like a good argument for improving health care.)

Yep. 'White collar' jobs generally have lower injury & death rates then 'blue collar' jobs. Absense of health insurance disproportionately impacts health outcomes in lower wage brackets.

Incidentially... white women (compared to white men) probably exceed the average difference in age-of-death that exists between 'blacks' and 'whites'. Would not your arguments apply equally in this case? Do women 'disproportionately benefit' from the program compared to men? Do white collar workers?
