I'm having trouble determining what exactly is the enormous harm of allowing them to continue to care for her.

There are two harms that come to mind. One is the harm to Terri on the off chance that she has some self awareness. It would be torture to be trapped in a dead body--to, say, have an itch you can't scratch not to be able to communicate with anyone, to be bored with nothing but your own thoughts for a lifetime. The other is the harm to the rule of law, one of the foundations of our society, when we arbitrarily make an exception rather than abiding by due process. Those have to be weighed those against the harm to the parents.
