The problem in the long run is that a surprisingly small number of people dying spectacularly and unexpectedly although with continued regularity, causes places to fall apart

Then Iraq fell apart some time before 2003, since they had 35 years of this condition. Saddam's rule depending on unexpectedly killing in fairly large numbers.

If you want to pinpoint the real cause of the problems with post-war occupation, there it is. Nobody outside Iraq had the real picture of just how broken the place was and how traumatized its people were.
