If the enemy has a substantial safe haven where they can easily escape to, and where they can be re-supplied its pretty hard to defeat them. The safe haven need not be a substantial piece of geography. That is a mistake our Army is currently deluding themselves with. The "safe haven" of a successful insurgency need not be much more than the hearts & minds of the participants. If enough of the Iraqi's hate the American "occupation" we will never win. American leaders & military need to wake up to this fact sooner rather than later.
