One item that has to be kept in mind is the lost earning potential resulting from going to college, grad or professional school and possibly residency.

4 years college + 4 years medical school + 2 or more years of low-paying residency = a lot of lost income if the person otherwise could be gainfully employed.

Now, if we all lived to 150 and were productive most of those years, maybe this wouldn't make much difference. But that ain't the case.
