I'll try to answer all your questions...
first of all, both of these technologies are just here until something better comes along. Carticel and Synovisc are not related treatments. Carticel is used to treat ISOLATED cartilage damage to one small area of the knee. These patients are typically young and active and need to be treated operatively. Synovic is used simply to prolong the inevitable joint repalcement procedure in older patients with global, all encompassing arthritic changes. These patients are not candidates for chondrocyte transplants.

Genzyme annoyed us by how they announced their results. They ran to NEJM ( a non orthopaedic journal) in an attempt to catch as many eyes as possible. They never allowed an Orthopaedic peer reviewed journal to review their data until almost 2 years after that paper. then at a major meeting they were touting their product as a cure for arthritis, which it is NOT. Orthopods are very skeptical and did not appreciate their marketing to the public and in effect slamming the proceudre down our throats. Then they were incredibly selective in determining who they would train to perform the procedures. It was an ugly few years, but they've put a lot of effort into making things right... although alot of skepticism is still about.

There are procedures coming on the horizon where the harvest will actualy be done from stem cells from the bone marrow, skeletal muscle , etc. This is a needle biopsy in the office.

MRI can not reliably diagnose these isolated defects because some are just too small and take us by surprise when we see them.

There are arthroscopic procedures also coming down the pike in the nexst ten or so years. Some are in development now (including Genzyme).

These stocks will run on hype for a while. they may even prodcue some profits, but as they stand now, their days are numbered. A few years at best.
