As a former federal employee, you know a lot about different health care coverage just because of what's available to you, and maybe also because you were in management.

The HMO/PPO model costs less and provides more than Blue Cross-Blue Shield type coverage. How can this be?

If you have the wrong name, you are operating in the wrong paradigm and you are trying to solve a problem with one hand tied behind your back.

Why do you say HMO/PPO isn't "insurance"? It's risk pooling. You pay the same for coverage whether you're sick or well, and it's there for you when you need it. It's just no frills.

Blue Cross-Blue Shield used to pay more for preventive maintenance but had to cut it out. My perception is that the model where the patient can get anything she wants and see whomever she wants whenever she wants is untenable. It's a "free rider on the commons" problem (my own mixed metaphor that I think describes it well).
