Cummulative rates will tend to understate mortatlity. This was seen in Sars, where the expanding denominator of new cases was much larger than the delayed numerator of reported deaths.

In addition to the delay caused be the disease course, there tend to be delays in reporting deaths, since these need to be "officially recorded and reported"

There was considerable spin on SARS on this.

Expect it on bird flu.
