It's when people--either the listener or the speaker--think the exaggeration is truth. This is the kind of sloppy thinking that is fostered by an environment with a lot of shrill polarization. I suppose we can debate whether or not he should have used that word or coined a different word, but that's what he was talking about. In which our disagreement is semantic.

If our disagreement were purely semantic, I would without question defer to you.

It is the example of sloppy thinking and shrill polarization where we disagree.
