I wonder if 200 just puts in some default max. I don't know that monitors can handle that type of refresh rate. Some don't handle 75. Its even possible to damage your monitor from giving it a refresh rate that it can't handle, however many drivers either lock out some rates or require you to explicitly change things to show unsupported rates.
