Here's my understanding of how it works:

A Market Maker acts as a middleman handling most trading transactions for NASDAQ stocks. Both the buys and the sells are recorded (as two separate events) in the volume statistics.

For example, lets say I want to sell 100 shares of EDHD and you want to buy 100 shares of EDHD. EDHD volume would be recorded as 200 shares because the Market Maker was involved and there were two transactions as a result.
