Andy, Targretin has shown potential for preventing a number of serious diseases (breast cancer, type II diabetes, lung cancer, cervical cancer, other types of metaplasia) so stan has raised the possibilty of Targretin (or other rexinoids which are vitamin-A derivatives) being used on a preventive basis, as vitamins are used.

Such an application would require chronic use by healthy individuals (or at least by those that are at risk for anny of the above). All drugs have a certain risk reward ratio. For many cancers, its relatively easy to tip the ratio into the reward category because failure to treat has such dire consequences. For healthy individuals, the reward side is less obvious and a small risk could tip the scales against use.

This is what happened to Tamoxifen for the prevention of breast cancer. It is currently the hormonal treatment of choice for treating breast cancer and it has potential for preventing breast cancer. A large trial was begun on patients at risk for developing breast cancer, but it was hlated because of an unacceptably high incidence of endometrial cancer. The relative low incidence was acceptable for patients with breast cancer, but not for those merely at risk for gettinng breast cancer.

Thus stan is paying more attention to side effects to see if they would preclude use of Targretin (or other rexinoid) in an at risk, but healthy population.
