First off, since marriage has always been defined as the union of man and woman in all known historic cultures (and I challenge anyone to show otherwise), gay marriage is a radical change in and of itself.

Second, most of the same-sex marriage advocates I've asked said they agreed that polygamy, incestuous marriages, group marriages would and should follow from changing the current rules on who can marry whom. That makes the proposal even more radical.

Third, the eagerness with which the gay rights movement has targeted the Boy Scouts (over a suit brought by a childless gay man who wanted to be a scout leader) and the Salvation Army (over gay employees), shows that the movement will target traditional socially beneficial institutions for demonization and legal sanction. Legalizing same sex marriage will strengthen this effort. I have no doubt that churches will be next and tax deductability of donations and tax exemptions will be challenged. Another radical result which will follow.

Lastly, I will note that same-sex marriages will not have to be gay marriages. There is no legal requirement that people who marry have to be sexually intimate, love one another, or live together. These are all social expectations and that's all. So one could marry relatives or friends to allow benefits to be extended to them. As the idea of what marriage is supposed to be changes, it will evolve more and more into a sham. In fact, I suspect that the vast majority of gay people have NO desire to marry for the traditional reasons. I suspect that a desire for more health benefits to be driving a lot of the current push for "gay marraige".
