Can you provide an example of federally upheld discrimination against a group with a physical/medical/biological disadvantage, apart from the view that homosexuality is innate and not chosen?

I can't recall any violations of equal protection for sufficiently compelling interests. Because equal protection is a constitutional principle rather than an expedient luxury. It is upheld in the face of other compelling interests because we value the principle in and of itself.
