Not to tell tales out of school, but I do know someone who used to work in a hospice, and that person was willing to "push the envelope" when it came to the line between killing pain and killing the patient.

This person explained that the patient would make their wishes known.

Nevertheless, I would rather that the action was the patient's action. There can't be very many situations where a patient is incapable of self-administering enough morphine via a morphine pump to suppress breathing if that is what the patient wants. All it takes is one finger, toe, or a turn of the head if the device were set up to allow it.
