It appears you are not correct on Mother Rock (I'll look into the rest, the latter ones may indeed have been long..):

All reports that I have seen indicate that Mother Rock miscalculated the volatility not the rise or fall of NGas prices.

It should be noted that they closed after there was an unexpected spike UP in prices in late July early August, not down:
