Now it could be that the media filters this, but when you read about a big malpractice award, the doctor in question often has a history of malpractice in other states. So having some centralized database that isn't in the hands of the doctors themselves would help a lot.

Ideally this sounds nice, but realistically the opposite would take place. Instead of keeping central accountability of doctors, the AMA (or whatever replaces it) will be given just as much power as, say, teachers unions. And teachers unions are notorious for rewarding seniority over performance.
