"Americans go to India cause it is cheaper, Canadians go to India because treatment is available."

Americans with health insurance don't go to India. The Americans without any health insurance that DO go to India go because without that cheap treatment option, they'd get NO treatment, as they couldn't afford it here in the states.

Their choice is slightly different than the Canadians, who could wait for treatment. The Americans could wait until they died, NO ONE would pay for their treatment.
