Blue Cross/Blue Shield employs an increasing army of people (just like every other US private insurance company) whose job is to deny paying for medical treatment to its insured by digging up old medical records "proving" that the treatment was a result of "preexisting" medical conditions, or the treatment was not necessary, etc. In fact, US private insurers spend from 20 to 30% (depending on the study) of every revenue dollar in screening and denying bureaucracy instead of health care. That's why in the last few years employment in US health insurance companies has exploded, while the number of their customers has not increased.
