I had in mind more of a reverse deductive process; after the Manila plot and the first WTC bomb, someone focuses the various parts of the bureaucracy to say OK, how do we make damn sure the Manila vision never comes true, and our planes never become bombs.

The scheme required hijacked planes and someone to fly them so focusing intel resources there and mandating hard doors would have been reasonable, if someone had take up the cause. It would be interesting to learn why the Clinton administration failed to consider or take those steps if they were considered.

The likely participants in any attack would have been non-US Muslims who have to come in through immigration channels. Tighten those up ASAP.

Like I said, it's all very clear in hindsight but I am not sure it fails a reasonableness test. My copy of Richard Clarke's book is packed away in a shipment I won't see for a month or I would go look it up to see if he mentioned those issues.
