Go look at the income statement of any large health insurance company. Deduct the amount of net profit, most of the amount spent on sales and marketing, and reduce the admin. costs.
The dollar amount you have left would be the actual cost of paying out benefits, i.e. covering the medical expenses of that company's policyholders.

My unscientific guess is that figure would be about 30% less than the bottom line on the income statement.
