Looks like the "nanny state" tried to deal with Cho. His danger was recognized and the university and legal sytem tried to get him mental health help. But it still failed to prevent his rampage. A person who doesn't really want help can't be made to take advantage of it. There is no fail-safe defense that will prevent someone from doing something like this if they want.

Cho was in control of himself and his attack was conscious and deliberate. He planned the attack over a lengthy period of time. Wrote an 8 page note behind in his room filled with rage at religion (apparently Christianity), rich people, debauchery, females, (who knows the extent of his hatreds) and claiming falsely, "You made me do this". Now we know he mailed photos, video, and writings to NBC ahead of the attack to be received afterward.

Described by everyone as a loner and depressed, he seems not to have been shy or withdrawn, but hateful and hostile. A chillingly evil young man.
