Webster, I've personally gone to a couple dinners specifically set up to market to doctors, and occasionally to pharmacists, too, and in those cases, I went as a spouse. There are many, many more that are doctors only. Half those in attendance were spouses, but mostly doctors. The vast majority are doctors only (although they often bring their spouses).

The two that I went to were "talks" given by other doctors who were sponsored by the drug company to research and also give presentations about clinical results and so on. The doctors giving the talk can easily pull down 100k or more a year. The doctors being marketed to eat dinner at the best places in town with free drinks too.

So maybe you are looking at a technicality -- the pharma pays the doctor to do the study, and then pays them very very well to talk to other doctors about the results. Seemed pretty direct to me though.
