You're right, I think, if I am understanding you correctly. Maybe all the good innovative stuff is to be found at the genesis stage. I spent some time trying to figure out what giant, established programs had innovated and done some real good. I couldn't think of anything. That doesn't mean I don't think some old programs do some good (like medicare)- I just don't think you can innovate much from the inside of a large bureaucracy (although you can start new programs off on the right foot, as Koop imo did)- and I think that's what you are saying. Am I hearing you right?
