Yes, you are understanding me correctly. And there are exceptions. In fact, you mentioned the one remarkable exception that always comes to my mind: Medicare. Their claims paying system was horrific for decades. Now that most of the "paperwork" is done electronically, the average time between claim and payment is a couple of weeks instead of a few months. It is contracted out, but that was true when it was almost all paper.

It seems to me that, whether it is a large government or a large business, the bureaucracy ultimately assumes a life of its own and looks after its interests first. That said, these days we are witnessing accelerated life-cycles of corporations/technologies (genesis, mature and might-as-well-be-terminal) within our lifetimes, something that was rare before.
