When I read your explanation, which is certainly clear, I understand why people get so confused.

It took some time for me to understand. The booklet the government sent me was the classic boondoggle. The insurance peddlers are not much better. They read from a script that I'm sure was sent to D.C. for approval before it was allowed for public consumption.

Bureaucratic overkill of the first magnitude in communicating a simple concept results in misunderstanding and Medicare is a classic example.
