It does mentioned it, but it doesn't go in to it in detail, or provide figures or estimates

You might have only one plan if the government took over but you would still have the same number of patients all with their unique needs and problems. And, would Medicare just be able to write checks if it were the only payer? Not likely. Medicare would be in the managed care business with all its bureaucracy before too long."

OTOH it also doesn't measure any extra efficacy that might be generated by the drive for profit. (At least not in direct terms but that could be considered a major reason why costs aren't so much higher without the economies of scale that Medicare has)
