Not always, and esp. not always for poorer people. I'm not poor, but I've personally gotten breaks on medical services that insurance didn't cover. I know other people that have as well. Discounts off the official rate are far from rare. I've never gotten hospital care (other than when I was born, which I don't remember and didn't pay for), so it might be different there.
