Some do, generally not the person who is responsible for the financial success, but the descendants of that person. Again, government bureaucrats tend to have better insurance policies than 'creators of wealth'.

"I would expect those who can afford it to seek out the best regardless of insurance coverage."

In my experience this is generally true of people who become financially successful due to events outside of their own control. Successful entrepreneurs are generally frugal people who evaluate the costs and benefits very closely. People who run companies and take them from small to large without support from their parents or other powerful and wealthy mentors tend to be frugal. Later in life after they have achieved significant success their frugality eases, but never goes away. Their children tend to be much more free with money and tend to enter adulthood at a much higher status than the creators of wealth.
