Waits for organs usually have little to do with who is paying the bill. There is a shortage of available organs. One reason for such a shortage is compensation for the organ donor (or the estate of the organ donor for organs harvested after death) is typically either not allowed, or very limited.

I don't know exactly how Europe, Canada, and Japan deal with this issue, but I don't think it is usually all that different than what we do. I'm pretty sure they usually also have long waits for organs, and if they don't it has little or nothing to do with issues of "universal health care", "single payer", or "a national health care system".
