We are all deciders indirectly. When we choose an insurance plan, we decide to accept its criteria and give tacit approval to them for use on others. If we go with a single-payer system, we will decide which elected representatives will be the deciders regarding the criteria. We play a role in determining the contract, in the former, and the consensus, in the latter.

But you ether do it for all or none.

Yes, but what's the "it." I agree that those sharing a contract or a government program should be treated equally under the criteria, but what are the criteria? There will always be rationing so there will always be criteria. The trick is to pick an appropriate "poison," as you called it. Do we do everything medically possible for every patient? Surely not. So, if it's not "the bottom line of the insurance company" but rather the bottom of the taxpayers' pockets, do we pay for experimental treatments or treatments unlikely to work or not?
