Through a variety of mechanisms consumers can discern the true coverage of health insurers. To the best of my knowledge all states have departments of insurance. They maintain files on every company that applies for the right to sell insurance in their state. When the file gets too thick or when egregious abuses occur the department investigates. Inquiries to the department are responded to with truthful information, such as "we have over 200 cases under investigation against them." In California that might not be too much but in North Dakota it would cause great concern.

When a person is going through group insurance the administrator is charged with disseminating information and answering questions like yours. As a former group administrator I can tell you that coverages and history of claims administration problems played into the decision of which insurance carrier to use. Cost is important but if costs are even close the hassle becomes more important. Coverages were an important factor in my choice or insurer.

People do compare service levels of everything. I often look online or ask people their experience before buying anything durable. I found out a fair amount about Blue Cross coverages and plans by discussing them with a neighbor who had it though his employer. That insight helped me make evaluations years later.

Just for curiosity, do you invest? Where do you get information? I get most of mine on line through brokers and places like SI. There seems to be a great deal of information available and the challenge becomes weighting sources for relevance.
