I haven't read any discussion on this, but one thing that occurred to me would be a staggered in payment of benefits type structure. For example, maybe have Medicare only cover catastrophic illnesses for the first 5 or 6 years after retirement. Have all but the very poor continue paying for their own visits to the doctor for ordinary illnesses, flu shots, mammograms, physicals, etc. etc. Then maybe Medicare would pay $500 per year toward those expenses for a few more years. I'm sure someone out there has explored this idea, and I would think the savings to medicare would be enormous, but I'm just guessing.
