I remember reading in the Wall Street Journal a front page article about for profit LA hospitals refusing to accept patients and sending the ambulance to the county hospital instead. That was only ten or fifteen years ago.

"there are now urgent-care centers to handle these minor medical conditions."

I am a good patient for the Doc In The Box as I refer to them. Some are excellent doctors that wanted to practice sooner rather than go through lengthy additional training and residency.
