Regardless of where the medical research is conducted the pharmaceutical company deriving the majority of its profits from American consumers makes the whole deal a free rider on American consumers. Until a reasonable number of foreign nations pay enough for their medications that US consumers are not forced to support the whole world I will continue to complain about them being free riders.

I will offer this caveat, due to a broken fifty year old approval system often other nations approve drugs years ahead of the US so there are times when drug companies have to suffice on the meager offerings from other countries. I suspect that the discounts offered at these times are not as deep.
